Before & After 3D Zygoma Reduction Surgery
The 3D zygoma reduction surgery offered by iWELL Plastic Surgery involves rotating and moving the cheekbones inward and reducing the thickness of the bones, thereby changing the appearance of the front and side cheekbones. The zygoma reduction helps make the face appear smaller, while enhancing its three-dimensional look.

BEFORE
– Strong, masculine look
– Wide and large face
– Looks older than one’s actual age

AFTER
– Looks older than one’s actual age
– Smoother jawline and slimmer face
– More youthful appearance
Top 3 Concerns Related to Zygoma Reduction Surgery

iWELL Plastic Surgery has eliminated
the top 3 concerns related to zygoma reduction surgery
1. Minimization of adverse effects through minimal incision and perfect fixation
High-quality titanium plates or absorbable plates are used for double fixation to ease the concerns regarding nonunion. Surgical instruments developed specifically for zygoma reduction by iWELL are used to minimize tissue separation and boost surgical accuracy, thereby easing concerns regarding sagging of the cheeks.
2. Smaller face that looks more three-dimensional!
iWELL’s 3D zygoma reduction surgery involves rotating and moving the cheekbones inward, unlike the methods applied in conventional zygoma reduction surgery. Through this method, the arched part of the cheekbones is moved inward, which helps decrease the width of the face as much as possible.
3. Speedy return to everyday routines!
An endoscope is used to view the surgical site during surgery, which helps enhance accuracy and minimize bleeding. Tiny incisions are made to reduce swelling and cause minimal scarring so that you can return to your everyday routines much more quickly.
Characteristics of iWELL 3D Zygoma Reduction
A face that looks most beautiful is one that is symmetrical with smooth contours. However, most people (more than 90%) have asymmetrical faces. Although facial asymmetry cannot be completely corrected, iWELL’s 3D zygoma reduction surgery can help improve the appearance and make the face appear more balanced. When it comes to cheekbones, even a tiny difference of 1~2mm can result in big changes, and this is why the plastic surgeons at iWELL perform surgery with accuracy and meticulousness to produce natural-looking results.

Small and Youthful Looking 3D Face
iWELL’s 3D zygoma reduction surgery is aimed at enhancing the volume in certain areas of the face, instead of simply pushing in the cheekbones, and this helps create facial contours that appear natural from every angle.

Plump Front Cheekbone Area
The front cheekbones are reduced in size in balance with the side cheekbones,
yet a plump look is created to make the face appear smaller and more youthful.

Reduction of Side Cheekbones
The width of the face is reduced so that the cheekbones do not
appear to be protruding outward to the side.

Cases of 3D Zygoma Reduction Surgery

3D Zygoma Reduction Surgery

The above patient had an asymmetrical face and the 45° cheekbones protruded outward prior to surgery. 3D zygoma reduction surgery was performed to address the facial asymmetry and the side and 45° cheekbones were reduced in size to make her face appear smaller and slimmer with a softer, feminine appearance.

3D Zygoma Reduction Surgery

The above patient had excessively well-developed cheekbones. To address this issue, 3D zygoma reduction surgery was performed to rotate and move the cheekbones inward and the width of the side and 45° cheekbones was reduced to create softer facial contours.

3D Zygoma Reduction Surgery

The above patient had a relatively wide facial bone on the left side of the face. To rectify this, 3D zygoma reduction was performed to create symmetry between the left and right sides of the face, which helped make the face appear slimmer with smoother contours from the front

3D Zygoma Reduction Surgery

The above patient had a relatively more prominent bone structure on the left side of the face compared to the right side. To address this issue, facial symmetry was restored through 3D zygoma reduction surgery. Also, the side and 45° cheekbones were reduced in size so as to improve the facial contours and create a softer look.

3D Zygoma Reduction Surgery/Chin Moved Forward

The above patient had excessively well-developed cheekbones and had a chin implant that made her chin appear long without addressing the short chin problem. For the cheekbones, 3D zygoma reduction surgery was performed to create symmetry between the left and right sides of the face, while the chin implant was removed followed by chin length reduction and sliding genioplasty (chin is moved forward). Through these procedures, a softer, more balanced looking face was achieved.
3D Zygoma Reduction Surgery Surgery Procedure

01 | A 3cm incision is made inside the mouth and a part of the front cheekbones is cut in a wedge shape. The extent of the cut is determined based on the size of the protruding cheekbones. |
---|

02 | The bone cut in a wedge shape is removed. (In most cases, a certain amount of bone is removed, but if this is unnecessary because the cheekbones are not excessively large, then the bones will only be cut.) |
---|

03 | A 1cm incision is made in the scalp near the ears to perform an osteotomy on the zygoma arch. |
---|

04 | The cheekbone is separated and rotated inward before it is fixed in place. |
---|

05 | The roughness of the boundary between the bones is minimized and a titanium plate or absorbable plate of the highest quality is used in double fixation. |

Osteotomy Principles of iWELL Plastic Surgery’s 3D Zygoma Reduction Surgery

Conventional Method of Zygomatic Osteotomy | iWELL’s Method of Zygomatic Osteotomy | |
---|---|---|
Osteotomy Site |
![]() |
![]() |
Osteotomy Method | L-shaped osteotomy | Wedge-type L-shaped osteotomy (prevents sagging and depression in the cheeks and facilitates union) |
Reduction Effect | General reduction of the face size | Osteotomy is performed in a deeper place than in the conventional method, so the face size reduction effect is further enhanced, and volume is added to certain areas for a more youthful face with well-defined contours |
Volumizing Effect | Small volumizing effect due to little improvement in the front cheekbones | Because the area of protrusion differs from individual to individual, a 3D-CT scan of the cheekbones is obtained so that volume can be added to wherever it is needed by identifying the MMP points. |
Rotation Principles of iWELL Plastic Surgery’s 3D Zygoma Reduction Surgery

iWELL’s 3D zygoma reduction surgery does not merely involve pushing in the cheekbones, but it serves to rotate the cheekbones inward, which effectively reduces the size of the side and 45° cheekbones. When both of the side and 45° cheekbones protrude outward, the balance of the face is greatly affected by the degree to which the cheekbones are rotated and moved. iWELL’s 3D zygoma reduction method is designed to rotate and move the cheekbones in a definite way, thereby effectively making the front, side and 45° cheekbones appear less prominent and restore balance in the face for satisfying results.
Fixation Principles of iWELL’s 3D Zygoma Reduction

The most important step in zygoma reduction is moving the cheekbones to an ideal location and securely fixing them in place. Unless they are fixed appropriately, there may be a chance of recurrence, or the cheekbones may move downward, causing a depression in the cheekbone area or the cheeks to sag. In such cases, the patient may also experience pain and hear a sound when chewing food. iWELL’s 3D zygoma reduction surgery involves the use of high-quality titanium plates or absorbable plates for double fixation of the cheekbones, which serves to minimize adverse effects such as malunion or nonunion. As a result, you can expect to gain more beautiful facial contours from this safe and effective zygoma reduction procedure.
