Corrective Breast Surgery

Corrective breast surgery is performed after breast augmentation for various reasons such as dissatisfaction with the breast size or shape, capsular contracture, implant migration or rupture, and asymmetry. The method applied in corrective surgery depends on the cause of the problem, and thus the cause must be accurately analyzed before proceeding with the procedure. This is why it is very important to seek a highly experienced plastic surgeon for corrective surgery.

When to Schedule for Corrective Breast Surgery

Best Time for Corrective Breast Surgery

Generally, it is best to schedule for corrective breast surgery 6 to 12 months after the breast augmentation procedure because it typically takes around 6 months for the breast tissue and shape to stop undergoing changes. However, it may be scheduled earlier in case of serious adverse effects. So we recommend that you consult a plastic surgeon to see what would be best in your case.

Reasons for Corrective Breast Surgery

In order to prevent another corrective surgery, it is important that the exact cause of the problem that has necessitated corrective surgery needs to be identified through a thorough analysis. Also, adequate consultation is essential in knowing what to expect after corrective surgery, and this is the most certain way to raise satisfaction and prevent failures.

case 05Damage to or deformation of implants

case 06Wishing to change to a different type of implants

Dissatisfaction with the shape

– Double Folds

This refers to the case in which another fold forms underneath the breasts in which implants are located. This may occur when sagging breasts were mitigated through just breast augmentation or surgery was performed without an accurate prediction of the resulting breast size.

– Dissatisfaction with the Shape

The most ideal results of breast augmentation surgery would be natural-looking breasts in the shape of a water drop, rather than breasts that bulge in the upper area. Bulging upper breasts result when the implants are place in inappropriate locations.

Method of Surgery
Partial correction may be possible in the early stages depending on the patient’s condition. However, during the middle and later stages, tissue separation is performed to move the implants into a more appropriate place or to remove the implants and insert new ones into an appropriate place.

Dissatisfaction with the Size

When choosing the implant size, you must consider your height and physique, and the conditions of the thorax among various other factors. Otherwise, you will likely be dissatisfied with the size and seek corrective surgery.

Method of Surgery
It is best to schedule the corrective procedure after a sufficient amount of time has passed (approx. after 6 months). This is a corrective breast surgery procedure with the lowest degree of difficulty. The original incision line is used to replace the implants with new ones.

Asymmetric Breasts

An in-depth examination is performed so that the existing implants can be replaced with the ones that are more appropriate in size for the patient. The new implants are also placed in the most appropriate location.

Method of Surgery
An in-depth examination is performed to determine the right implant size for the patient and replace the old ones. The new implants are inserted into an appropriate position and shaped afterwards.

Damage to Implants

The cohesive gel implants that are being used lately have a more robust outer layer compared to the conventional implants, which helps lower the chance of rupture. In the case of liquid silicone and saline breast implants, on the other hand, there is a high chance of rupture and the contents may leak and cause deformation.

Method of Surgery
If the implants are saline-filled implants, there won’t be a problem. However, in the case of silicone implants, the area must be washed thoroughly to completely remove the silicone gel that has leaked out. Then, the capsules are removed and new implants are inserted into the breasts.

Capsular Contracture

This refers to a condition in which surgically enhanced breasts become round and hard like baseballs. This is a condition that occurs when the membrane forming around the breast implants thicken and cause the breasts to harden and change in shape. There are diverse causes of capsular contracture, but the most common ones are inflammation and hematomas (collection of blood) resulting from hemorrhaging. The method applied to address capsular contracture depends on its stage.

Levels of Capsular Contracture Surgical Method
① Grade 1: Breasts feel soft and natural and the symptoms cannot be felt by touch.
② Grade 2: There are no visible symptoms, but the breasts feel less soft and the implants can be slightly felt.
③ Grade 3: The breasts are firm and the implants can be easily felt. The symptoms are visibly noticeable.
④ Grade 4: The breasts are hard and misshapen, which is clearly apparent. This is accompanied by pain.
case of capsular contracture, which is the most common reason for corrective breast surgery, drug therapy Is Recommended For/To grades 1 and 2 and corrective surgery is necessary for grades 3 and 4.

Principles of iWELL’s Surgery for Capsular Contracture

The epithelium surrounding the implants is completely removed. (Capsular contracture cannot be addressed if the epithelium is not removed or only partially removed.)

A new space is created for new implants, while making sure that they do not come into contact with the previous surgical site. (The double plane method is implemented to insert the implants.)

Artificial skin is grafted into the area if the skin is too thin.

iWELL Corrective Breast Surgery Using HD Endoscope

At iWELL Plastic Surgery, HD endoscope equipment is used in corrective breast surgery. It is used to flawlessly remove the existing implants and to insert the new implants in the exact locations, while ensuring minimal incision and precise tissue separation. This helps greatly reduce pain and bleeding and create beautiful breasts.

iWELL’s Corrective Breast Surgery System

You need to be extra prudent when it comes to receiving corrective breast surgery. Satisfying results can be ensured through an accurate assessment of the problems and application of the best surgical method for the noted problems. Choosing a plastic surgeon with abundant experience is important when it comes to breast augmentation surgery, but it is especially important for corrective breast surgery.

Corrective Breast Surgery System STEP 01

In-depth Examination Prior to Surgery

Breast surgery is a highly difficult type of surgery that requires the patient to be placed under general anesthesia.
At iWELL, we perform pre-op tests and exams (blood test, ECG, chest X-ray, breast ultrasound, etc.) to ensure safety. Through this process, we analyze the patient’s condition in depth and proceed with a plan that is strictly tailored to the patient in question.

Corrective Breast Surgery System STEP 02

Pain Alleviation Based on Intercostal Nerve Block

A local anesthesia is administered for the intercostal nerves, which convey information on the pain felt in the chest to the brain, as a way to minimize the pain experienced by the patient during and after surgery. At iWELL, general anesthesia and intercoastal nerve block are administered at the same time to reduce pain and facilitate a speedy recovery after surgery.

Solution for Preventing Capsular Contracture

The biggest causes of capsular contracture are infection and hemorrhaging. We have introduced an aseptic system to practice surgical aseptic techniques so as to significantly reduce the chance of capsular contracture. Also, during surgery, hemostasis methods are thoroughly applied to stop the bleeding and blood bags are used to prevent pooling of blood. On top of that, an anti-adhesive agent is used to minimize capsular formation in order to prevent capsular contracture caused by bleeding. Furthermore, a wide view is secured using an HD endoscope and a headlamp when checking the nerves, blood vessels, muscles, etc. in the chest area so that precise and accurate tissue separation can be performed to prevent any unnecessary pain.

Reliable Safety System

We have acquired the ISO 9001 (Quality Management System) certification by conforming to the international quality management standards for the plastic surgery service area. Also, with patient safety as our No. 1 priority, we have a full-time anesthesiologist specializing in plastic surgery who provides care to patients throughout their stay from preparation for surgery to recovery after surgery. We are also equipped with an uninterruptible power supply (UPS) system so that electric power can be supplied to the surgical equipment even in the event of a power failure. We also have emergency equipment and an automated external defibrillator (AED) in the event of an emergency as well as an air shower system for eliminating particulate matter and microorganisms in the air, UV sterilization system, and double door system to block out contaminants. We also provide safety training to our staff on a regular basis.
